How to Register as a Person with a Disability (PWD)

In-Person Submission

Obtain the necessary paperwork

Application forms are available at the Sub-County Gender & Social Development Offices.

These documents consist of:

  1. Medical Assessment report from the Government Gazetted Hospitals
  2. Copy of Passport/ ID card
  3.  Copy of Birth Certificate
  4. Proof of lawful residence in Kenya
  5.  Copy of parent’s/legal guardian certificate of registration
  6. Certificate of good conduct for children above 18 Years
  7.  A copy of the certificate of police clearance

To Apply Online:

First, submit an online application.

  1. Copy of Passport
  2.  Copy of Birth Certificate
  3. Proof of lawful residence in Kenya
  4.  Copy of parent’s/legal guardian certificate of registration
  5. Medical certificate on disability
  6.  Certificate of good conduct for children above 18 Years
  7. Applicants must ensure that they are lawfully present in the country as they wait for application to be finalized
  8.  Copy of the Police Clearance Certificate

You must visit an immigration office in Nairobi, Mombasa, or Kisumu, or a Kenyan mission abroad, after printing the application and invoice, to pick them up and submit them for processing.

The following paperwork needs to be sent to the offices.

You will receive your PWD information right away after submitting your registration application.

  3. Duly filled individual registration form
  4. A printed online application form 12 (Print the form after completing your application)
  5. Copy of Passport
  6. Copy of Birth Certificate
  7. Proof of lawful residence in Kenya
  8. Copy of parent’s/legal guardian certificate of registration
  9. Certificate of good conduct for children above 18 Years
  10. Copy of the Police Clearance Certificate
